Death Certificate
A death certificate is a confidential document, therefore, only the immediate family – the spouse, the biological or adoptive parents, the biological or adoptive grandparents, the legal guardian, the siblings, or the adult children of the deceased can request and obtain a death certificate that shows the cause of death.
If you have a tangible interest in a matter pertaining to the decedent, you can request a certified copy of the death certificate; however, the cause of death will be redacted.
To obtain a death certificate you must present a government issued picture I.D. (driver's license, military I.D., state issued I.D., or passport).
You may obtain a death certificate in person or by mail by visiting or writing the Probate Court. Download the request form here.

Requests by mail must include the following information: the full name of the deceased, date of death, county of death, and county of residence. Please include a check or money order and a self-addressed, stamped envelope. Also, a clear, legible photo copy of government picture I.D. is required.
The cost of obtaining a Peach County death certificate is $25 per person for the first certified copy and $5. for each additional certified copy.
The Probate Court accepts cash, credit card (Visa, Discover, MasterCard), personal check or money order payable to the Peach County Probate Court. The Probate Court will not accept business or third party checks.
